Named GP Policy

In accordance with NHS guidelines and our commitment to providing personalised care, every registered patient at our practice is allocated a Named Accountable GP. This GP will have overall responsibility for overseeing your care and coordinating with other health and social care providers involved in your treatment. The aim of this policy is to ensure continuity and consistency in managing your health needs.

Important Points to Note

Having a Named GP does not restrict your access to other doctors or healthcare professionals within the practice. You are welcome to see any of our doctors or nurses, as all clinicians in our practice have access to your medical records and can support your health needs.

• Your Named GP will not be available at all times, and in some cases, another doctor may be better placed to assist you based on availability or urgency of care. Please be assured that this policy does not affect your ability to book appointments as you currently do.
• If you would like to request a particular doctor as your Named GP, please inform a member of our reception team. We will make every effort to accommodate your preference, though there may be occasions where this is not possible due to operational requirements or workload balance.

For Patients Aged 75 and Over

• Patients aged 75 and over have an allocated Named GP who also takes a lead role in coordinating a multi-disciplinary care package, where relevant, with other health and social care professionals. This GP ensures that you have access to an annual health check as stipulated in NHS guidelines.

If you are unsure of who your Named GP is, you may ask at reception during your next visit to the practice. There is no need to telephone the practice specifically for this information. New patients will be informed of their Named GP at the time of registration.
